Quote Requirements

Risk Description
Completed GL Applications including:
- Membership Total (fitness clubs)
- Total Number of Trainers
Total Participant Enrollment (amateur sports/gymnastics)
- Total number of coaches
Completed Equipment Section
- Birthday Parties, Swimming Pools, Saunas, Tanning Beds, Etc.
Total Revenue
- Retail Sales included separately
- Concessions, café, smoothie bar revenue included separately
Optional Coverage section completed.
- Include limits for SAM, Professional Liability, HNOA, Umbrella
5 years hard copy loss runs (or as far back as business inception)
Business Plan/Revenue Projection for new ventures.
Copy of Employee Handbook
- For SAM limits greater than $100,000 please include abuse prevention procedures
Acord 140 required for any risk that is requesting property coverage
- Include COPE information such as construction type, # of stories, year built and total square footage
